Your Privacy Matters

As registered physical therapists, Peak to Prairie Physiotherapy is committed to protecting the privacy and confidentiality of you personal health information in accordance with applicable Canadian privacy legislation, including the Personal Information Protection and Electronic Documents Act (PIPEDA) and The Health Information Act (HIA) of Alberta.

This Notice describes how your health information is collected, used, and disclosed for the purposes of care, education, payment, health care operations, and other purposes permitted or required by law. It also outlines your rights regarding your information.

Personal Health Information

Personal health information includes any information (including demographic information) that identifies you and relates to your past, present, or future physical or mental health, the provision of health care services, or payment for those services.

Consent and Use of Information

Your personal Health Information may be collected, used, and disclosed with your consent, or as permitted or required by law, for the purpose of providing care, supporting your health goals, and operating the practice of Peak to Prairie Physiotherapy.

Peak to Prairie Physiotherapy will obtain your verbal consent before sharing your personal health information with other health care providers unless disclosure is otherwise permitted or required by law.

I understand that my personal health information may be used:

(a) to provide care, education, and support services;
(b) to support continuity of care within the “circle of care,” which may include sharing information with other regulated health professionals involved in my care, with my consent;
(c) to bill and receive payment for services, where applicable;
(d) to deliver, evaluate, and improve services;
(e) for practice management purposes, including planning, quality improvement, monitoring, and reporting;
(f) to establish, maintain, and securely store accurate health records;
(g) to comply with legal and regulatory requirements; and
(h) as otherwise permitted or required under applicable federal or provincial laws.

Disclosure of Information

I understand that my personal health information may be disclosed, with my consent or as permitted by law, to:

(a) other regulated health professionals involved in my care;
(b) The Mama Coach Inc. or its team members, where necessary to support administrative functions or care coordination, and with my consent;
(c) hospitals or clinics providing treatment to me, upon request; and
(d) any other party where disclosure is permitted or required by law.

Where personal health information is used for education or training purposes, it will be de-identified whenever possible, or shared only with your consent.

Safeguards

Peak to Prairie Physiotherapy takes reasonable administrative, technical, and physical safeguards to protect your personal health information from unauthorized access, use, or disclosure.

Your Rights

You have the right to:

- request access to your personal health information;

- request corrections to your personal health information;

- request information about how your personal health information has been used or disclosed, where applicable;

- request restrictions on certain uses or disclosures (subject to legal and operational limitations);

- receive communications about your personal health information in a confidential manner;

- withdraw consent for the use or disclosure of your, where permitted by law; and

- file a complaint with The College of Physiotherapists of Alberta, or with the Office of the Privacy Commissioner of Canada or your applicable provincial privacy regulator if you believe your privacy rights have been violated.
